Community Support Staff

Within a community based residential setting, Community Support Staff (CSS) uses best practices and technology to teach people with developmental disabilities how to acquire the physical, intellectual, emotional and social skills needed to live as independently as possible.


Essential Functions (Summarized):

-Providing assistance to individuals, as necessary with daily living skills and by doing so developing independent community skills.

-Utilizes Person Centered Plans to evaluate member’s needs and interest in the community, and to develop goals and objectives.

-Assists in the development of a weekly schedule for each member to maximize their time spent in the community.

-Completes training with individuals towards achieving ISP goals.

-Administers medication(s) and completes appropriate documentation, where necessary.

-Accompanies individual to and from appointments and activities.

-Assists in building a support network between the individuals and community members.

-Accurately completes all in-house documentation: maintains current case notes, and monthly reports of individual’s progress.

-Participates in Circle(s) of Support for personal futures planning.

 

 

 

Senior Community Support Staff

The Senior CSS  uses best practices and technology within a community based residential setting to teach people with developmental disabilities how to acquire the physical, intellectual, emotional and social skills needed to live as independently as possible. Performs all duties necessary to meet state-approved objectives and requirements. Maintains a high quality program in accordance with Company standards.


Essential Functions (Summarized):

-Acts as supervisor in the absence on the CSS Assistant Manager or CSS Manager.

- Completes training with individuals towards achieving (Individual Habilitation Plan) I.H.P. goals.

-Administers medication(s)  and completes appropriate documentation

-Accurately completes all in-house documentation.

-Provides reports on consumer's progress and how it relates to overall goals.

-Participates in Circle(s) of Support for personal futures planning.

High School Diploma or equivalent required along with one (1) year experience in related field.
Valid Driver's License with a good driving record is a must

 

 

Community Support Manager

Community Support Manager (CSM) manages the community support staff. Within a community based residential setting, the CSM uses best practices and technology to teach people with developmental disabilities how to acquire the physical, intellectual, emotional and social skills needed to live as independently as possible.


Essential Functions (Summarized):

Assists in maintaining and monitoring of individual finances.

Monitors and reports health and medical needs of individuals.

Monitors and schedules house and vehicle maintenance.

Builds a support network between the individuals and community members.

Monitors fiscal, medical, personal and IHP documentation to ensure accuracy.

May perform the duties of Community Support Staff as necessary, in accordance with current staffing needs.

Prepares progress reports for consumer's immediate support group, including their family and/or guardian, staff, employment specialist, case manager, and other professionals.

Ensures consistent service delivery that connects work, home and recreational needs.

Completes staff evaluations.

Monitors and schedules required staff training's, seminars, and conferences.

Submits monthly written reports to the appropriate individuals outlining operational status, progress, and concerns.

Monitors and maintains residential budget.

 

 

Program Manager

The Program Manager uses best practices & technology within a therapeutic day program setting to teach people with developmental disabilities how to acquire the physical, intellectual, emotional & social skills needed to live as independently as possible. Ingenuity & creativity must be utilized when determining the best training methods, assistance & supports for an individual. Knowledge and understanding of local regulatory agency operations; Understanding and commitment to community support for persons with disabilities; Flexibility to deal with people of a variety of levels; Oral and written communication skills a plus; Excellent time management a must.


Essential Functions (Summarized):

Provides direction to services coordinators and job coaches who assist our clients with competitive employment placement and maintaining that employment.

Teaches the employee independent job skills and requirements, utilizing job task analysis and job specific supports, including assistive technology and personal assistance.

Provides assistance to services coordinators and job coaches in reinforcing essential skills, or teaching new skills appropriate to developing a resume, conducting a successful job interview, and completing a job application.

Instructs services coordinators and job coaches how to work side by side with the newly placed individual at the job site, analyze the job, and break it into manageable components.

Identifies and solves problems before they become a crisis for the consumer, employer or employees.

Oversees and supervises a support network between the clients and community members.

Accurately completes all company documentation in a timely manner.

Maintains accurate participant files and records, including information on case histories, work performance, activities, written reports, correspondence, ESP’s, narratives, and case notes.

Develops, identifies, and promotes job openings in the community. Assists other team members in developing appropriate jobs for clients. Maintains contacts with employers to insure their needs are being met.

 

Coordinator of Residential Services

The Coordinator of Residential Services maintains a high quality program in accordance with the mission statement and national office standards of Community Options, Inc., i.e.: small homes, personalized supports, supported employment and person-centered planning. This individual must be able to handle the pressure of training in a public environment, exercise patience, work with a flexible schedule, and respond to a variety of situations. Ingenuity and creativity must be utilized when determining the best training methods, assistance, and supports for an individual.


ESSENTIAL FUNCTIONS (Summarized):

- Builds a support network between the individuals and community members.

- Prepares progress reports for member's immediate support group, including their family and/or guardian, staff, employment specialist, case manager, and other professionals.

- Ensures consistent service delivery that connects work, home and recreational needs.

- Serves as an active member of each person’s Inter-Disciplinary Team (IDT), which includes involvement in Individual Habilitation Plan development.

- Submits monthly written reports to the appropriate individuals outlining operational status, progress, and concerns.

- May perform the duties of Community Support Staff as necessary, in accordance with current staffing needs.

- Works collaboratively with the Service Coordinators.

- Presents problems and recommends any necessary changes in current policy and procedures to the Executive Director.

- Makes recommendations to the Executive Director for the development of current and future programs.

- Monitors fiscal, medical, personal and Individual Habilitation Plan (IHP) documentation to ensure accuracy.

- Participates in Circle(s) of Support for personal future planning.

- Monitors and reports health and medical needs of individuals.

- Assists in the maintaining and monitoring of members finances.

 

Option Quest Coordinator

The Option Quest Coordinator is responsible for developing and implementing all Option Quest activities, assisting participants in scheduled and regular community activities, and/or volunteer, educational or pre-vocational programs.


ESSENTIAL FUNCTIONS (Summarized):

Meets with Executive Director, employment staff and residential staff regularly to determine which individuals can benefit from Option Quest services.

Assists Executive Director in obtaining D.D.D. eligibility for those individuals.

Meets with individuals, family members, case managers, guardians, residential staff and other interested parties to determine appropriate Option Quest activities.

Prepares weekly schedule of appropriate activities for each Option Quest participant.

Schedules support staff to assist individuals with their activities, and helps staff as necessary.

Maintains written case notes, and submits monthly written reports to Executive Director on all Option Quest activities.

Assures that all daily logs are properly recorded.

 

 

 

Employment Specialist

The Employment Specialist uses best practices and technology within a community based employment setting to supervise and train people with developmental disabilities on how to acquire the physical, intellectual, emotional and social skills needed to perform specific job tasks, work routine, and personal life activities critical to employment success. Using the least intrusive methods possible on the job, their objective is to gradually reduce the time spent at the job site, as the individual becomes better adjusted and more independent.  Performs all duties necessary to meet state-approved objectives and requirements.  Maintains a high quality program in accordance with Company standards and mission.


ESSENTIAL FUNCTIONS (Summarized):

Provides the necessary assistance for the individual to be placed in competitive employment and maintain employment.

Teaches the employee independent job skills and requirements utilizing job task analysis and job specific supports, including assistive technology and personal assistance.

Assists individuals in reinforcing essential skills, or teaching new skills appropriate to developing a resume, conducting a successful job interview, and completing a job application.

Works side by side with newly placed individual at the job site, analyzing the job and breaking it into manageable components.

Executive Director

The Executive Director maintains a high quality program in accordance with the mission statement and national office standards of Community Options, Inc., i.e.: small homes, personalized supports, supported employment and person-centered planning.


ESSENTIAL FUNCTIONS (Summarized):

Responsible for all daily operations and for compliance with state licensing and reporting standards.

Formulates and implements an advisory board which will assist the organization, and develop and expand their resources, thereby improving their service delivery.

Presents a clear picture of issues related to their local operations to the Regional Vice President and/or the National office, outlining possible solutions and indicating advantages and disadvantages.

Recommends new approaches, policies, and procedures to effect continual improvements in efficiency of department and services performed.

Obtains information from professional sources to augment available knowledge.

Locates and recommends appropriate grant and funding opportunities.

Produces fundraising activities that will effectively promote the needs of the national organization, and increase their funding base.

Promotes and maintains a positive public image of Community Options, Inc., in their state and local area.

Develops, implements, supervises and coordinates all office procedures, programs and services for their location.

Recommends sound personnel practices in employee recruitment, evaluations, salary, termination and employee benefits.

Institutes and maintains appropriate management, financial and personnel controls, and provides the national office with the necessary documentation and paperwork.

Prepares required statistical, financial and service reports as needed.

Interfaces with local, state and governmental agencies.

Participates in developing long and short-term department goals, objectives, and systems.

 

 

Assistant Executive Director

The Assistant Executive Director works with the Executive Director to ensure that programs are effective and operate in accordance with the mission statement and national office standards of Community Options, Inc., i.e.: small homes, personalized supports, supported employment and person-centered planning.


ESSENTIAL FUNCTIONS (Summarized):

Fills in for the Executive Director in their absence.
Assists the Executive Director with all daily operations.

Responsible for compliance with state licensing and reporting standards.

Assists Executive Director in developing a Business Advisory Committee in their area to help find jobs for people with disabilities and further develop Company resources.

Presents problems confronting their local agency to the Executive Director, outlining possible solutions.

Recommends any necessary changes in current policy and procedures to the Executive Director.

Works in conjunction with the Executive Director to locate and recommend appropriate grant and funding opportunities to the national office.

Assists the Executive Director with the development and implementation of fundraising activities.

Assists the Executive Director with employment related issues, including the preparation of the necessary paperwork and documentation.

Makes recommendations to the Executive Director for the development of current and future programs.

Promotes and maintains the public image of Community Options, Inc., in their state and local area.

Establishes and maintains effective relationships with local, state and governmental agencies.

May perform the duties of Community Support Staff as necessary, in accordance with current staffing needs.
